Preheat the oven to 375°F (190°C).
In a large bowl, mix ground chicken, breadcrumbs, Parmesan cheese, mozzarella cheese, and garlic together.
Add milk, Italian seasoning, salt, and black pepper to the mixture, and combine thoroughly.
Form the mixture into a loaf shape and place it in a greased loaf pan or on a baking sheet.
Spread ketchup on top of the formed meatloaf, if desired.
Bake in the preheated oven for about 50-60 minutes, or until the internal temperature reaches 165°F (74°C).
Let the meatloaf rest for 10 minutes before slicing.
